It's easy to add your child to our waiting list! If you've called or visited and been told we're on a wait for your child's age group, please fill out one of the forms above.

Choose "Electronic" for an MS Word application to submit via email to info@dhcdc.org.

Choose "Print" to hand-deliver or mail an application.

Enrollment Packett

If you already know there is an opening for your child's age, please fill out a copy of the application above, and bring it to any of our locations along with:

The child's original birth certificate
An up-to-date physical, signed by the physician
A current shot record, also signed by the physician
Either the parents' W-2 form(s) or four most recent pay stubs
